FINANCIAL OPTIONS
FEE FOR SERVICE
We are a fee for service Dental Practice. Patient portion is due at the time of service. Claims are submitted the same day electronically. Once we receive EOB (evidence of benefits) from your insurance if there is a remaining balance you will receive a statement.
As a service, we will provide you with a treatment plan along with an estimate of what your insurance will pay and estimate of your responsible portion.

INSURANCE
If you are covered by dental insurance we will gladly accept and file for you as long as you can select a dentist of your choice. We work with most dental insurance carriers, from national providers to locally-based providers here in Colorado; we are a preferred provider for Delta Dental Premier.
We will gladly process your insurance claim, estimate your deductible and the portion not covered by your insurance. The estimated amount not covered by your insurance is due at the time the treatment is done and may be paid by anyone of the options listed below. Our estimates are subject to final approval by your insurance company; therefore, the amount due to our office is subject to change.
Our front office staff will verify your insurance benefits prior to your dental appointment.
NO INSURANCE
Our office requires payment at the time of treatment. A quote will be given prior to treatment. A treatment plan will be provided and will list all necessary treatment and amount due at the time of service.
PAYMENT OPTIONS
Cash: includes money orders and personal checks.
Visa/ Mastercard/ Discover/ American Express: we accept credit cards as payment for treatment to the extent your credit limit permits.
CareCredit: offers a separate line of credit to cover your entire family's health care needs.
CREDIT LINE
